We thank you for your support and participation in 2014. We pray that your family and church will receive many blessings in the New Year of 2015.
In regards to filling out your tax report for 2014 you must be enrolled in an insurance company or healthcare sharing ministry in order not to be penalized. When you submit your tax report you have to fill out the ‘8965 Form’ and send that in as well. Below is an example guide of the ‘8965 Form’ for your reference.
We verified that Kim, James(3 family participants) have been CMM members from 04/01/2014 to current date.

2014년 세금보고와 관련하여 일부 면제자를 제외하고 (1)보험이나 (2)Health Care Sharing Ministry에 가입되지 않은 분들은 벌금이 발생하게 됩니다. 회원님들께서는 세금 보고 시 ‘8965 Form’을 작성 하셔야 하므로 아래의 샘플 사항을 참조하시기 바랍니다. Kim, James (가명) 가족 3명은 04/01/2014부터 현재까지CMM 회원으로 등록되어 있습니다.
