We sincerely thank you for your past participation as a member in our Health Care Sharing Ministry.

 

Pursuant to your membership cancellation request, you are asked to submit a completed Cancellation Request Form. Or, click Cancellation Request Form to submit the completed form via email, fax, postal mail, or text. If the completed Cancellation Request Form is submitted by the end of the month, the membership cancellation will be effective on the 1st day of the following month.

 

Please call us at 773-777-8889 (Ext. 5003) to check whether you have any pending submitted medical bills. 

Note: any submitted medical bills will not be shared regardless of the date of the medical service once you cancel the membership.

 

그동안 의료비 나눔 사역에 참여해주셔서 진심으로 감사드립니다. 

 

회원의 탈퇴 요청에 따른 탈퇴 절차가 완료되려면, 아래 Cancellation Request Form을 작성하든지, 

탈퇴 신청서를 눌러 작성한 후 이메일, 팩스, 우편 또는 문자로 보내 주셔야 합니다. 매달 말일까지 탈퇴 신청서가 접수되면 다음 달 1일부터 탈퇴 처리됩니다.

 

본인이 제출하여 진행 중인 의료비가 있는지 확인하려면 773-777-8889 (Ext. 5003)으로 전화 주십시오. 

노트: 탈퇴로 인해 멤버십이 중지될 시, 치료받은 날짜에 상관없이 제출된 의료비는 지원되지 않음을 유념하시기 바랍니다.

*필독: ① If ‘YES’, the member needs to maintain the membership for 30-60 days at the current level after ALL requested documents are received completely. (Yes일 경우, 의료비 지원 서류가 완전하게 접수된 후 30-60일까지는 같은 레벨로 회원을 유지해야 합니다)

② When the cancellation is completed, medical bills in progress, on pending, and/or with outstanding balance will not be shared. (탈퇴하면 진행중이거나 보류 또는 미접수된 의료비는 지원되지 않습니다)
