Before visiting a Medical Facility

  • Planning Your Visit:
    • A member is asked to inform the CMM Needs Processing Department 4-5 days prior to their scheduled medical procedure or visiting a healthcare provider. In case of emergencies, the member is asked to call with 48 hours to receive necessary instructions. 773-777-8889
  • Check Eligibility:
    • The range of shared medical expenses may vary depending on the level you choose, how long you've been enrolled, and whether you have any pre-join conditions.
  • Required documentation from hospital visits: 
    • As an uninsured patient, we will guide you on the next steps, including what documents are required and how to communicate with your healthcare provider (such as requesting an itemized bill or providing proof of payment).
    • You may also request intercessory prayer for your health and recovery.

When visiting a healthcare provider

  • Enroll as a Self-Pay Patient:
    • CMM is not health insurance. When registering at a hospital, enroll as a Self-Pay or Uninsured patient.
  • Request Required Documents: Ask the healthcare provider for the following:
  • If a healthcare provider applies a discount or adjustment and it is documented on the medical bill, the discounted amount may be deducted from your personal responsibility.

After your visit to a healthcare provider

  • Apply for medical assistance:
    • Review and verify the information provided by your healthcare provider.
    • If you have a hospitalization, surgery, or a large medical bill, do not pay it immediately. Contact the Needs Processing Department to discuss your situation and receive guidance.
    • Prepare your medical assistance application and supporting materials.
  • Submit Required Documents
    • Submit the required documents to the CMM Needs Processing Department to be eligible for sharing of the medical expenses. Keep the original documents for your records and submit copies only.

Application Filing
and Procedures

  • Make sure all required documents are complete and organized to avoid delays due to missing or insufficient information.
  • Once all requested documentation is received, processing typically takes approximately 60 days.
  • You will receive an email notification confirming receipt of your documents.
  • We may contact you by phone regarding important matters during the medical assistance process. Please add 773-777-8889 to your contacts so you do not miss important calls.
